Transitions of Care Pilot Studies
ACMA convened case management leaders from across the care continuum to develop the Transitions of Care Standards to be applicable across all care settings. In 2019, we conducted early demonstration projects with organizations recommended by the TOC Executive Steering committee. The case studies highlight their story.
The Transitions of Care demonstration project/case studies were designed as short-term pilots, intended to test the feasibility of implementing the standards and consensus measures. Although the sample size is small, this early/preliminary information is helpful in the next steps for learning.
In 2020, a six-month Transitions of Care Learning Collaborative began, with participation from six major organizations: Advocate Aurora, Duke Health, Inova Health, Memorial Hermann, Trinity Health-St. Francis Hospital & Medical Center and Tenet Health.
TOC assessment data will be analyzed from these organizations and findings presented through a White paper and ACMA conference presentations.
